SCC Covid-19 Self-isolation Survey 2021


Dear Somerset resident,

Hello - I'm Alison, the Public Health lead for Covid19 in Somerset sending this message to you on behalf of Somerset's Director of Public Health, Professor Trudi Grant.

We are asking for your help - having previously been diagnosed with Covid-19, we are very keen to learn more about your experiences with the virus:

  • how it has affected your health and wellbeing
  • the advice you were given following your diagnosis
  • the support, if any, you received at the time
  • any challenges you faced
  • recommendations you might have, based on your own experiences, to improve the experience for others, going forwards

This is a voluntary exercise, but it would be really helpful if as many Somerset residents as possible were able to complete and return a response to the following questions. You have had an experience unique to yourself and we are keen to learn from this.

Please answer every question honestly. We need to understand the challenges that have been faced and to look at trends across our population that require better support.

All data will be recorded anonymously. The survey responses will be analysed by the Public Health team within County Hall and whilst we hope to use a few of the quotes that people report to us, these will not be attributed to individuals.

Purpose of processing: to review and learn from the advice and support the Council provided to residents following a Covid-19 diagnosis.
Categories of personal data: in completing the survey we will collect your Contact Tracing Advisory service (CTAs) number and your postcode.
Legal basis for processing: processing is necessary for the purpose of the legitimate interests pursued by the controller (GDPR Article 6(1)(f)).
Data sharing: No personal data will be shared. Responses will analysed and only anonymous results shared.
Retention: individual responses we be retained for 6 months.
